Windsor concrete company fined $175,000 for fatality

WINDSOR, Ont. — A concrete manufacturer in Windsor, Ont., has been fined $175,000 for a workplace incident that killed one employee and injured another.
The Ministry of Labour says the fine was imposed Jan. 20 under the Occupational Health and Safety Act after Prestressed Systems Inc. pleaded guilty to failing to adequately protect a worker.
The ministry says the incident took place in April 2018, while employees worked on a concrete form for a large pre-stressed bridge girder.
It says an anchor plate slid from its supports and “catastrophically failed,” striking two workers, one of whom died.
The ministry says the company failed to follow requirements of the equipment’s manufacturer.
The court also imposed a 25-per-cent victim surcharge, which goes into a provincial fund for victims of crime.
